DESMOND – Diabetes Education

What is DESMOND?

DESMOND is a nationally approved education programme for people with Type 2 diabetes, whether newly diagnosed or living with the condition for longer.

It is designed to help you understand what Type 2 diabetes means for you and how to manage it day to day.

Sessions are friendly and informal, not like being back at school. You will learn practical tips on food choices, physical activity, and medication, and you can share experiences with others. You will be in a small group (up to 10 people) and can speak to an Educator privately if you wish. You are welcome to bring a partner, family member, carer, or friend.

Who is eligible?

DESMOND is free for adults with Type 2 diabetes who are registered at North Shore Surgery or Glenroyd Medical within Blackpool North Primary Care Network (Blackpool North PCN).

If you think you are eligible, you can:

  • Contact your practice via PATCHS (Admin or Other request – please mention DESMOND).
  • Call your practice and tell the receptionist you would like to join a DESMOND session.

Diabetes education can help you feel more confident managing your diabetes and living well with Type 2.

When and where are sessions held?

For North Shore Surgery and Glenroyd Medical Centre patients, DESMOND sessions are usually held on a Saturday at:

Moor Park Health & Leisure Centre

Bristol Avenue

Blackpool

FY2 0JG

Please contact your practice to find out the next available session.
