Are you planning to collect something from our reception?
Does what you’re collecting have ANY identifiable information on it? – (E.g. Name, Date of Birth, Address, NHS number, etc)
If so, we require ID for collection. If you are sending someone in on your behalf, you must inform the practice and specifically name who is coming in. This is to ensure the safety and security of your information. The person collecting will also be asked to produce ID to ensure we are handing over your information to the correct person
Have you been asked to provide a sample?
Please be aware that our cut-off time for receiving samples is 4 pm. This ensures that all samples are collected and taken away to the labs the same day they are given. We cannot hold up or ask the collection van to wait.
