If you have been referred by one of our Clinicians, you can expect to receive the necessary paperwork, usually within 1 week of your appointment. Your paperwork will contain both your password and details how to book your appointment. As this paperwork is not routinely posted, you will be contacted via text/phone to collect from your usual Surgery as soon as it is ready. If you are collecting paperwork from our Surgery on Whitegate Drive, please allow at least 2 days before collecting.
