These appointments are conducted by the Practice Nurse and are for patients with ongoing complaints that require more complex management plans to reduce the risk of complications, such as asthma, COPD, diabetes, heart disease, hypertension, and stroke.
We have a recall system in place, whereby our administrative teams invite patients to attend their annual chronic disease reviews based on their birth month throughout the year.
